USDA Nondiscrimination Statement

To file a program discrimination complaint, a Complainant should complete a Form AD-3027,

at: https://www.usda.gov/sites/default/files/documents/ad-3027.pdf, from any USDA office, by calling (833) 620-1071, or by writing a letter addressed to USDA. The letter must contain the complainant’s name, address, telephone number, and a written description of the alleged discriminatory action in sufficient detail to inform the Assistant Secretary for Civil Rights (ASCR) about the nature and date of an alleged civil rights violation. The completed AD-3027 form or letter must be submitted to:

Food Support Program
A program to provide certification of eligibility and issuance of food support to public assistance recipients and other needy clients according to U.S Department of Agriculture (USDA) regulations. This agency will accept collect calls regarding the SNAP program.

Minnesota Family Investment Program (MFIP)
During 1998 the state of Minnesota instituted Statewide MFIP as its welfare reform program with initiatives that expect, support, and reward work. MFIP replaces AFDC, AFDC Food Stamps, Family General Assistance, and STRIDE employment and training. Direct benefits include food stamps and cash assistance. MFIP clients may also receive Medical Assistance (MA), child care help, and employment services.

Minnesota Food Assistance Program (MFAP)

Minnesota Supplemental Aid (MSA)
A program to provide payments to aged, blind, or disabled persons to supplement available income up to the State's public assistance standards.

General Assistance (GA)
A program to provide cash payments to persons who do not qualify for federal categorical assistance but meet the State General Assistance requirements, including emergency payments.

Diversionary Work Program (DWP)
The Diversionary Work Program is a short term, work focused program. It includes intensive, up front services to focus on a family’s strengths and break down barriers to work. Minnesota counties have implemented DWP since July 1, 2004.
